Medically complex family begs for assistance to save their house

Hello,
We are a non traditional family of 6. My husband and I have 3 biological children which 2 have disabilities, and have had guardianship of my sister with Down syndrome since 2016. My husband and I are in our early 30’s. My husband has stage 4 lung disease and is not able to work as of now. He is denied social security the past 6 years due to his age.
I have been in school for oocupational therapy and the only income for our household besides our 2 children’s disability income. Some months I make an extra $100 and social security cuts it from the check. We have been in this viscous cycle since my husband fell ill in 2018. My parents are deceased since I was young and I was never taught how to invest or save to be economically stable. I am eager to make changes, learn, and end this vicious cycle. Unfortunately the past several years have be tumultuous for our family. We also had an at birth diagnosis of down syndrome for our son in 2018 which had led to more medical attention and therapies than our family already had. We had my husband’s unexpected hospital stays and diagnosis of Sarcoidosis shortly there after. Our eldest son has ADHD and Tourette and requires lots of assistance in daily living. My younger sister who is 27 with down syndrome requires total assistance in all areas of daily living. I am only one person and have no family support. I have recently had caregiver burn out and dealing with autoimmune issues but putting medical issues on the back burner.
In the mean time I have been struggling to meet my mortgage, had to go on forbearance and now have to figure a way to pay 20,000 or raise my mortgage from 2,100 which I struggled to pay already to 2,600. My credit cards are maxed from just trying to feed my family and put gas in our cars to get to medical appointments and school.
My goal is to get out of debt, get off of social security once I get my state license (which I can’t afford the rest at the moment), and open my own pediatric therapy company within my community. I do not want to stay stuck on public assistance and refuse to be in this position again in the future.
Any assistance would be greatly appreciated and forever a positive impact on the future of my family.
